Why incomplete submissions cost everyone time

Most delays in trucking insurance are not underwriting delays. They are information delays: a missing VIN, a driver without a license number, a loss run that covers two years instead of three. Each gap becomes an email, and each email adds days. A complete first submission is the single biggest lever a producer has on turnaround time.

The list below reflects what carriers using Undivo can require in a published intake configuration. Your carrier may ask for more or less, but these items cover what most programs ask for.

Company identity and operations

  • Legal business name, DBA, entity type and FEIN.
  • USDOT and MC numbers, with the physical and mailing addresses matching the FMCSA record.
  • Years in business, primary commodities, hazmat status and operating states.
  • Radius of operation, annual mileage and garaging locations.
  • Primary contact with a working email and phone. Company officers from the census record are a good starting point.

Fleet schedule

  • Every power unit with VIN, year, make, model, gross vehicle weight rating and ownership status.
  • Trailers listed separately, only where the applicant owns or is responsible for them.
  • Declared totals that reconcile with the schedule. If they do not match, explain why.
  • Stated values for physical damage coverage and any lienholders.

Drivers

  • Full name, date of birth, license number, state and class, and years of commercial driving experience.
  • Motor vehicle record violations and accidents for the period the carrier requires.
  • A driver count that matches the roster, or a written acknowledgment where the carrier permits a deferred roster.

Coverage and history

  • Requested lines with limits and deductibles: auto liability, general liability, motor truck cargo, physical damage, non-trucking liability, trailer interchange, umbrella or excess, and workers’ compensation where applicable.
  • Current and prior insurers with policy numbers and effective dates for at least three years.
  • Currently valued loss runs for the same period, attached to the policy record they describe.
  • Certificate holders and additional insureds that must appear on the certificate at binding.

Acknowledgments and signature

The applicant should review every section before signing and understand what the signature confirms. In Undivo the signature is recorded against a frozen copy of the application and the carrier’s wording, so a material change after signing requires a fresh acceptance. Producers can send a signing link to the applicant instead of collecting a wet signature.

Once the application is submitted, the next step should be visible to everyone: an estimate, an information request or carrier review. That clarity is the difference between a submission that moves and one that waits.
